Contact & identity information: name, email address, phone number, mailing address, date of birth

Service records: intake forms, application details, assessment information, program history

Billing & payment data: payment method, transaction history, invoice records

Communications & consent data: messages, opt-in / opt-out records, SMS logs

Technical & usage data: IP address, browser type, device identifiers, pages visited, form submissions

Health / sensitive information: only when explicitly provided for program purposes (e.g. medical history, medications). If you provide health or behavioral health data, it is treated with heightened confidentiality under applicable laws (e.g. 42 CFR Part 2 and HIPAA where applicable).

Staff members receive training on confidentiality and data protection
